1. All clients will receive a copy of the HHS summary of their rights.
2. All employees will comply with the Privacy Policy.

4. The information in your records is confidential and will be disclosed only as required or permitted by law unless YOU authorize us to release it to some other party. Written authorization may be faxed to 716-625-1323. You may also deliver the authorization to us in person or by mailing it to the address above.
5. We may release information to another health care professional with whom we have consulted on behalf of you or your child; the collaborating physician(s) Dr. Chester Fox and, in his absence, Dr. Chris Occhino, or their designated covering physician; Public Health Authorities, where required by law; as ordered by a court; or to the parents of a minor (with certain legal exceptions: e.g. reproductive health.)
6. Our paper and computer records are locked when the office is closed.
7. This office is operated within the confines of a church. The employees and volunteers at the church are all aware of the importance of confidentiality of any information to which they may inadvertently gain knowledge.
